When I was trying to get a YYF spacer off of a bearing, and somehow ending up recking the bearing. 
Oh, and missing a GT suicide with a responsive yoyo, ouch. :o
i was poping out of a green triangle and i hit myself right in the tooth and it partially hit my braces, it dinged my 44 and it hurt really bad.
about 4 months ago the yoyo came around that missed my finger and hit my light glasse came down on me.
me and my family were having a barbeque on a rainy day with my neighbor who i am teaching to yoyo. so it starts drizzling and i hear thunder. i dont take this into account, as i am doing eli hops with : a metal yoyo. it was a genesis by the way. nothing happened but i felt sooo stupid after this happened, especially since i was standing in like an inch of water. ( it was like a broken rubber tarp.) all i have to say is that it would have hurt!
Every time I hit myself in the face with my speed maker, because it’s quieter, at work and have to go check in the mirror to make sure I don’t have to fabricate a black eye story for my boss.

When I was trying to learn boing-e-boing way back when, when I got the bouncing started, the yoyo slipped out of the two strings it was bouncing between. Hit me right in the face. And let me say, it hurt more than you would expect.

well… I went to a yoyo meet today… nailed myself in the eye with a DM… does that count?
i dinged a new raw vs newtion battosi at ser… and it was not mine…
