Need No Jive Axle Sleeve Dimensions

I need to make some axle sleeves for someone. The trouble is, I’ve lost the sheet where I wrote down all the dimensions.

I’m not liking my luck here, but is there anyone here who has an No Jive axle sleeve and a pair of caliper to measure the dimensions of one for me?
